Availability Days-In order to process your application we require you to provide days and times available to volunteer. We require a minimum 4 shifts per month. If these times do not work please provide alternates in the box below.

Waiver of Liability
This agreement releases Hope for Wildlife from all liability relating to injuries that may occur while volunteering with Hope for Wildlife. By providing your consent above, you agree that Hope for Wildlife is free from any liability, including financial responsibility for injuries incurred.
You also acknowledge the risks involved in volunteering with Hope for Wildlife. These may include but are not limited to: wildlife induced injury (bites, scratches, etc.), contraction of disease, injury associated with capture, containment, and transport of wildlife. You swear that you are participating voluntarily, and that all risks have been made clear to you. Additionally, you do not have any conditions that will increase your likelihood of experiencing injuries while engaging in this activity.
By selecting your consent option above, you forfeit all right to bring a suit against Hope for Wildlife for any reason. In return, you will receive a volunteer position with Hope for Wildlife. You will also make every effort to obey safety precautions as outlined in the Dispatch Volunteer Manual and as explained to you verbally. You will ask for clarification when needed.

Consent of Personal Information Sharing
You acknowledge and agree to the sharing of the information provided among the Hope for Wildlife staff and members of the Hope for Wildlife Dispatch Team. This information is not required to volunteer with the Hope for Wildlife Dispatch Team but is collected to help the Hope for Wildlife Dispatch Team run more effectively.
You agree to share personal information as outlined below with Hope for Wildlife and the Hope for Wildlife Dispatch Team, starting on the date of submission of this application. You understand that this information is only to be used for the purposes of the Hope for Wildlife Dispatch Team. You acknowledge that as long as you are involved with the Hope for Wildlife Dispatch Team, your information will continue to be shared with its members. If you no longer wish to volunteer with the Hope for Wildlife Dispatch Team, you must provide Hope for Wildlife written notice to have your personal information removed from the Hope for Wildlife Dispatch Team and thus abstain the sharing of my information.

Hope for Wildlife confidential information may include, but is not limited to:
Specific patient information, or progress of a patient. This includes all animals that are NOT visible to the public.
Private information regarding staff, volunteers, interns or other organizations associated with Hope for Wildlife, including but not limited to any contact information or involvement with Hope for Wildlife.
Private information regarding Hope for Wildlife’s clients including but not limited to their involvement with Hope for Wildlife, their contact information, or any privately reported information.
Specifically, all information that is publicly accessible refers to information on Hope for Wildlife’s public displays (including free ranging birds and education animals), or posted on the Hope for Wildlife media platforms (i.e. the Hope for Wildlife Facebook page, and the Hope for Wildlife webpage). All other information is not to be shared publicly.
In this agreement the person(s) mentioned above are defined as:
Clients: any person(s) who have contacted Hope for Wildlife to report a wildlife incident (including, but not limited to injured, sick, or orphaned wild animals).
Staff: any person(s) directly employed by Hope for Wildlife.
Volunteers: any person(s) that assist in the function of Hope for Wildlife but are not directly employed.
Interns: any person(s) undertaking an internship at Hope for Wildlife.
Organizations associated with Hope for Wildlife: any group(s) or organization(s) that assist in the function of, or conducts work in association with, Hope for Wildlife in any way.
Members of the public: any person(s) who are not directly involved with Hope for Wildlife (i.e. not staff, volunteers, or clients).

Confidentiality Agreement
You agree with the following statements:
Not to disclose any confidential information, as outlined in Hope for Wildlife’s Privacy Policy, to members of the public or other volunteers unless explicitly instructed to do so by Hope for Wildlife staff members. This includes all information that is not publicly accessible regarding the activities and function of Hope for Wildlife.
Not to publicly post on any social media or elsewhere, any confidential information or photographs/videos containing confidential information as outlined in Hope for Wildlife’s Privacy Policy.
You understand that you may come in contact with confidential information during my time at Hope for Wildlife. As part of the condition to volunteer with Hope for Wildlife you hereby undertake to keep in strict confidence any private information as outlined by the Hope for Wildlife’s Privacy Policy.
